A 31-point daily pre-use checklist for boom lifts and scissor lifts. Mark OK, FIX or N/A on every item, sign it, and keep a dated record. Fill it in on screen or print it and use a pen. Instant download; print as many copies as you need for your own workplace.
About this checklist
Check the lift before anyone goes up. This one-page daily checklist walks the operator through 31 points in the order they actually inspect a machine: walkaround, fluids and power, ground controls, platform controls, the work area, and PPE before travel.
Every item gets OK, FIX or N/A. Any FIX means the lift isn't used until it's repaired or tagged out of service. The operator signs it, and a supervisor can sign off too.
The pack also includes a 7-day weekly log, for crews that want one sheet per machine per week.
Helps with OSHA 29 CFR 1910.67(c)(2)(i) and 1926.453(b)(2)(i): lift controls tested each day before use. The READ-ME explains how the sheet lines up with these and related rules. It is a checklist template: the employer remains responsible for meeting all requirements, following the machine maker's instructions, and training workers. Not legal advice.
US Letter size (8.5 x 11 in). Black-on-white sheets that print cleanly on an office printer.

Good to know
This is a checklist template, not legal advice. It doesn't replace the manufacturer's operating manual, your employer's safety program, or the training operators need. Follow the manual for your machine and mark N/A where an item doesn't apply.

FAQ
Does this checklist help with OSHA requirements? Yes. It helps with OSHA 29 CFR 1910.67(c)(2)(i) and 1926.453(b)(2)(i) for testing lift controls each day before use. A form can't do the job on its own: your company stays responsible for the requirements that apply to it, following the machine maker's instructions and training workers.
Which lifts does it cover? Boom lifts and scissor lifts. Mark items that don't apply to a machine as N/A.
Can I fill it in on a computer? Yes, in any PDF reader or a current browser such as Chrome, Edge or Firefox. Type your entries, then save a copy or print. You can also print the sheets and fill them in by pen.
What if my download link expires? The link stays active for 30 days. After that, contact us through the site with your order number and we can resend it.
