About these checklists

The attendant sheet has 24 checks, an entrant in / out log with 5 lines and 2 attendant relief lines, so the count of who is inside is always written down. The entrant sheet has 24 checks and a line for the entrant's own times in and out. Both sheets run in three sections: A, before entry; B, during entry; C, exit and emergency.

Tick Y or N on every line, and N/A only on lines that have an N/A box. An entrant with any N in section A does not enter and tells the attendant or the entry supervisor; the attendant tells the entry supervisor about any N before entry starts. The sheets set no gas limits: items that mention limits point to the acceptable entry conditions on the permit.

Written for OSHA's general industry rule for permit-required confined spaces, 29 CFR 1910.146. The attendant sheet helps with the attendant duties in 1910.146(i), and the entrant sheet helps with the entrant duties in 1910.146(h). The READ ME lists the paragraph behind each setup check.

This is a pre-entry checklist, not a permit. It does not replace the entry permit, your site's permit-required confined space program or the entry supervisor's authorization. It is a checklist template: it does not replace the equipment maker's manual or any rule that applies to your work, and some items may not apply to your space. Not legal advice.

FAQ

Does this replace the entry permit? No. It is a pre-entry checklist used alongside the permit. The permit, your site's program and the entry supervisor's authorization still apply.

Who fills in which sheet? The attendant fills in one attendant sheet for each entry or shift. Each entrant fills in an entrant sheet before going in. Keep the filled-in sheets with the permit if your site wants them kept.

Does it list gas limits? No. The acceptable entry conditions come from the permit for that space, and monitors are used the way the maker's manual says.

What if my download link expires? The link stays active for 30 days. After that, contact us through the site with your order number and we can resend it.
