About this log
Keep a simple, repeatable record of your daily forklift checks. The log is a set of reusable master sheets: give every truck an ID and print a fresh weekly sheet for each truck every week.
Each weekly check sheet is one double-sided sheet per truck, with 7 day columns and 33 numbered checks marked OK, D (defect) or N/A: key-off walk-around checks on the front, key-on checks and a supervisor weekly review on the back. Page 3 explains every checklist row in plain language. Running more than one shift? Use one day column per shift and start a new sheet when the columns fill.
When a check finds a problem, the "What to do about a defect" steps and the Defect & Repair Log help your crew handle it the same way every time: tag the truck out, report it, give it a D-number, and record the repair and the return to service. Truck records, an operator roster and a Maintenance & Service Record keep the rest of each truck's paperwork together.
This log helps with OSHA 29 CFR 1910.178 daily inspection requirements: 1910.178(q)(7) calls for trucks to be examined before being placed in service, at least daily, and after each shift where trucks run around the clock. Rule excerpts are quoted word for word from the eCFR (29 CFR 1910.178, version dated Oct 29, 2026). It covers electric and LP, gas and diesel forklifts and the other powered industrial trucks covered by 1910.178; it is not for the vehicles 1910.178(a)(1) excludes. It is a log template: the employer remains responsible for meeting all requirements, following the truck manufacturer's instructions, and training operators. Not legal advice.
US Letter size (8.5 x 11 in). Black-on-white sheets that print cleanly on an office printer.

FAQ
Does this log help with OSHA requirements? Yes. It helps with OSHA 29 CFR 1910.178 daily inspection requirements and includes word-for-word excerpts of the standard. A form can't do the job on its own: your company stays responsible for the requirements that apply to it, following the manufacturer's instructions and training operators.
Does it work for electric, propane and gas/diesel forklifts? Yes. There is one weekly check sheet for electric trucks and one for LP, gas and diesel trucks. Mark rows that don't apply as N/A.
We run more than one shift. Will it work? Yes. Use one day column per shift. Start a new weekly sheet when all 7 columns are used.
Can I fill it in on a computer? Yes, in any PDF reader or a current browser such as Chrome, Edge or Firefox. Type your entries, then save a copy or print. You can also print the sheets and fill them in by pen.
How many copies can I print? As many as your business needs for its own workplace(s). The license doesn't allow resale or sharing the files outside your company.
