About this pack
Keep one written PM record for every truck. The PM schedule page sets four PM levels, PM 1 to PM 4, from the hour meter or months, whichever comes first. Common steps of 250, 500, 1,000 and 2,000 hours are printed only as an example: write in your manual's hours and months for each level. A service does every checklist item at or below its level, so a PM 2 does the items marked 1 and 2.
The checklists are generic and fit most forklifts and pallet jacks: electric, LP, gas and diesel, sit-down and stand-up, manual and electric jacks. Some steps may not apply to your truck, so mark those N/A. Sections A to F are for all trucks, section G is for electric trucks only and section H is for LP, gas and diesel trucks only. Follow the manufacturer's manual for intervals, fluids, settings and anything a general checklist does not cover.
When a PM finds a repair, it goes on the repair & follow-up log with its own number (R-001 and up), with boxes for the notification number and the work order number. The log helps keep a written record of trucks removed from service and repairs, per OSHA 29 CFR 1910.178(q)(1). It is not a replacement for the daily pre-use check in 1910.178(q)(7): keep using a daily pre-use checklist for that. It is a maintenance record template: it does not replace the truck manufacturer's manual, your company's procedures or any rule that applies to your equipment. Not legal advice.
US Letter size (8.5 x 11 in). Every box is sized for a pen.

FAQ
Will it fit my forklifts? It fits most forklifts and pallet jacks: electric, LP, gas and diesel, sit-down and stand-up. Some steps may not apply to your truck; mark those N/A, and tick WHOLE SECTION N/A on the power section that does not fit.
Which PM intervals should I use? Your manual's. The 250, 500, 1,000 and 2,000 hour steps are only an example; write your manual's hours and months on the schedule page.
Does it include the daily operator check? No. It is not a replacement for the daily pre-use check in OSHA 29 CFR 1910.178(q)(7). Use a daily pre-use checklist, such as our Forklift Daily Inspection Log, for that.
What if my download link expires? The link stays active for 30 days. After that, contact us through the site with your order number and we can resend it.
