About these sheets
Record every incident and near miss the same way: what happened, where and when, who saw it, why it happened and what you are doing about it. The 2-page report covers injuries, illnesses, property damage, spills and near misses, with each Form 301 item labeled on the sheet. A witness statement, a 5-whys root-cause worksheet and a corrective-action tracker take each report through to a fix, and the monthly log keeps a running list of every event.
The 3-page guide explains how to use the sheets, when to call OSHA (a reporting box with the 8-hour and 24-hour reporting rules and the OSHA phone number), and how OSHA records and employee privacy work. Page 5 quotes the regulation excerpts word for word from the eCFR (29 CFR Part 1904 and 1910.178, up to date as of Oct 29, 2026).
Not a replacement for OSHA's 300, 300A and 301 recordkeeping forms. Our report is not an OSHA form or an equivalent form (29 CFR 1904.29(b)(4)). Not legal advice. Your company stays responsible for the recordkeeping and reporting rules that apply to it.
US Letter size (8.5 x 11 in). Black-on-white sheets that print cleanly on an office printer.
